These are offset printed in any of our standard ink colors and numbered in red ink. You can modify this form as you like.

2 part or 3 part carbonless paper.
PAPER SEQUENCE
-  2 part - White and canary.  3 part - White, canary and pink.

- Any of our stock ink colors. Black, Red, Dark Blue, Light Blue, Green, or Brown. Note ST-1329 comes in the colors as shown

LOGOS
- After you purchase your forms we will contact you for all the details.  If you plan to provide logos and/or artwork, please ensure that the file is "camera Ready" - a gray scale 300 dpi file in PDF, JPEG or Tiff formats. Line copy reproduces the best as screens, gradients and halftones tend to have some loss in detail. No bleeds are possible with this product. If you have a question about the file we will be glad to help you. An electronic proof will be sent to your email address for approval within 24-48 hours excluding weekends and holidays. Any changes in the body of the form may require additional proof time.
YOU ARE RESPONSIBLE FOR APPROVAL OF PROOFS, PROOF READING AND ERRORS.
We will keep your file so reorders will be fast and easy.
